Incident Reporting in Medicines Information System (IRMIS) was used prior to the launch of Active Learning from Events and Risk Tracking (ALERT). 

These PDF summary reports capture key themes, trends and learning identified from incidents reported through IRMIS and provide a valuable resource for shared learning, reflection and continuous quality improvement in medicines information and advice services.

To maintain confidentiality, detailed information about individual incidents is not included in these summaries. Detailed reports are only accessible through the Members Only area.
