During 2025, incident reporting transitioned from Incident Reporting in Medicines Information System (IRMIS) to the new Active Learning from Events and Risk Tracking (ALERT) system.
As a result, the Q1 2025 summary report includes learning from incidents reported through both systems before ALERT was fully implemented.
From Q2 2025 onwards, incidents have been reported through ALERT.
The PDF reports below provide an overview of key themes, trends and learning points identified through incident reporting. They are intended to support shared learning, reflection and continuous quality improvement for all staff involved in providing medicines information and advice.
